For the "error = 3", does it come from app "PIV Data Loader" ? If yes, please make sure only PIV Smart card driver (NIST SP 800-73 [PIV]) installed on your Windows PC (This driver will be installed via Windows Update automatically when you plug in the PIV Smart card.). That means do not install other drivers/app of Smart card, such as ActivClient, .NET card driver and Java card driver.
